We begin our journey together with an Assessment conducted by Patricia. She takes into account the specific needs of your loved one and your family, as well as the needs of the primary caregiver. Family dynamics can be tricky, so Patricia always presents a number of options and solutions. Drawing on her in-depth experience, she ensures that everyone’s safety concerns are met, even in difficult situations. She addresses emotionally charged situations so everyone feels safe. Mobility issues such as walkways, stairs and shower access are considered. Family, loved ones and caregivers experience an overall feeling of security, safety and predictability.

Based on the Assessment outcome, Patricia designs a Customized Care Plan for the personal circumstances of your loved one, your primary caregiver and your family members. She carefully matches you with the right team professional, someone with the experience and background you can trust.

Your Customized Care plan delivers a comprehensive, detailed blueprint for Daily Home Care Essentials, Enhanced Companion Care, Consultation & Advocacy, and End of Life Doula services. Your plan not only sets goals but also measures outcomes, so you always know what is happening.

Personal Support

We provide flexible solutions for your emerging needs

As your situation evolves, we change and adapt your Plan. We provide flexible, creative and friendly solutions for each emerging need. This may include monthly personal support sessions for the caregiver, as well as community and health care outreach engagement. Through our work, you and your loved one can continue to be part of your community.

  • Ongoing support may include:
    Private consultations for family members and their care givers.
  • Communication tools to deal with challenging behaviors such as not speaking, not communicating or isolating.
  • Sharing information on community health care networks.
